1. Define

We’ll start by getting to know your safety needs and your goals, as well as the “fuzzy” dynamics which are specific to your organization’s culture. From this information, we will design a bespoke survey to ensure we capture the right data we need to deliver your bespoke improvement plan.

Planning session – a video call to discuss your goals, identify your deployment champion and explain our processes.

Demographic questions – We will ask the questions that will best capture key data about different groups in your organization to enable deeper analysis and further insights.

Survey development timeline – We will calculate how best to promote the survey in your organization, getting the timing just right to ensure the best survey engagement and response rates.

Custom survey aesthetic – Your survey can use your company branding or ours, depending on your preference.

Survey construction – We will use the Fuzzy Systems online platform to carefully and securely build your survey.

Project charter – We will provide you with a summary of your Human Factors Survey System.

2. Measure

Using our advanced tools, we quantify and measure human factors. Our use of fuzzy logic allows us to capture nuanced data that reflects the true state of your organization’s safety culture.

Deployment best practice – You can use our recommended survey deployment plan with email templates to promote your survey and secure the best response rates.

Promotional flyer/poster – Print out and hand out our pre-written and designed poster/flyer to your people to serve as a reminder about the survey and encourage participation.

Survey QR code – We’ll supply a QR code you can use in your promotional material which your employees can scan with their cell phones. This will link directly from posters, flyers and emails to your survey.

Response rate reports – How many of your team have taken the survey? We’ll report back weekly with the stats so you can decide if more promotional activity is needed.

Survey data storage – We’ll provide the means to secure the data generated by your survey. Retaining this data is vital for the future comparative and trend analyses that are key to the Human Factors Survey.

3. Analyze

We delve into the data to uncover patterns, trends, and areas of concern. This analysis helps us understand the root causes of safety issues and how they relate to the overall organizational behavior. We transform your data into a comprehensive report full of information that will give you deep cultural insights into safety at your organization.
Your 30+ page report will include:

Executive summary – We will reveal your top 3 Areas of Strength and Top 3 Areas of Improvement.

The HFS polar map – This is a visualization of your human factors culture, showing your organization’s Dirty Dozen scores and whether they are strengths or areas in need of improvement.

Demographic analysis – You will receive visualizations and comparisons of the population subgroups identified in your demographic questions.

Diagnostic checkpoint – if your deployment is more complex, we will discuss your options for further detailed analysis if required.

Dirty Dozen in detail – for each of the 12 human factor elements, we’ll present:

  • Numeric score
  • Key takeaways
  • Areas of strength
  • Areas needing improvement
  • Noteworthy items
  • Employee voices Recommended improvements

4. Improve

We’re not the kind of people who’ll fly off into the sunset once our report is submitted. Ours is a long term commitment to you and your organization, with ongoing interactions to ensure your improvements are generating the changes you need to see. We develop tailored data-based improvement plans, designed to address specific safety concerns and enhance the overall safety culture within your organization.

Consultation call – We’ll discuss and document your improvement plans, goals and timeline.

Flight plan – We use the insights our work generates to create a bespoke Flight Plan for your organization. This is a strategic improvement plan which will give you actionable data-based recommendations to improve the three human factor elements that scored lowest in your Human Factors Survey. These will draw on best practices and interventions that have proved successful in situations similar to yours.

Intervention list – We’ll create a data-based table containing all the recommendations identified during the Analyse phase of the Human Factors Survey.

Delta manual – You will receive a comprehensive 20-page PDF guide that walks you through the DELTA Framework for Effective Intervention. It presents a hands-on approach to address the areas for improvement identified in the Analyze phase.

Delta workshop – We can host a two-day, in-person workshop consisting of a focus group of your employees who took the Human Factors Survey. The purpose of the sessions will be to help identify the best improvement projects for your team by leading your people through the DELTA Framework.

5. Control

We implement control mechanisms to ensure continuous improvement and keep your safety culture strong and responsive to new challenges.

Year 2
A follow-up survey a year after the initial survey enables us to analyse year-on-year changes to your Dirty Dozen. This will highlight areas of significant change, whether positive or negative, between the first and second deployments so you can gauge the effectiveness of your interventions.

Year 3
By this stage, we will have enough data to monitor cultural trends, track the direction in which your safety culture is evolving, and show your team’s trajectory over time.
